المحفزات

كيف يُشخَّص؟
يُشخَّص الصداع النصفي سريريًا، أي بالقصة المرضية والفحص العصبي — لا بالأشعة. والرنين المغناطيسي لا يُطلب لكل مريض؛ إنما يُطلب عند وجود علامات تحذيرية، أو فحص عصبي غير طبيعي، أو نمط صداع غير معتاد. ومن المهم أن تعرف أن خلوّ الأشعة من أي خلل لا يعني أن ألمك غير حقيقي — بل هو النتيجة المتوقّعة في الصداع النصفي.
علاج النوبة

العلاج الوقائي وحقن البوتوكس

الإفراط في المسكنات: المشكلة الخفية
الإكثار من مسكّنات الصداع قد يقلب صداعًا نصفيًا متقطّعًا إلى صداع شبه يومي، فيبدو الأمر وكأن المرض ساء بينما السبب هو الدواء نفسه. والقاعدة العملية: إذا كنت تحتاج مسكّنًا في أكثر من عشرة أيام شهريًا بشكل متكرر (أو أكثر من خمسة عشر يومًا للمسكنات البسيطة)، فأخبر طبيبك. والحل ليس زيادة الجرعة، بل خطة لسحب المسكّن تدريجيًا مع بدء علاج وقائي.

Migraine is a common neurological disorder that shows up as recurring attacks of headache — often on one side of the head, throbbing in character, and made worse by movement and exertion. How it is diagnosed
Migraine is diagnosed clinically — from the history and the neurological examination, not from a scan. An MRI is not ordered for every patient; it is requested when there are warning features, an abnormal neurological examination, or an unusual headache pattern. It matters to know that a scan showing nothing wrong does not mean your pain is not real — a normal scan is exactly what is expected in migraine.
Treating an attack
The single most important rule: treating early works far better. Take the medicine as the pain begins, not once it is fully established.
- Simple painkillers or anti-inflammatories for mild to moderate attacks.
- Triptans: designed specifically for migraine, used when ordinary painkillers are not enough. They are not suitable in certain heart and vascular conditions, so they are prescribed after assessment.
- Anti-nausea medication: eases the nausea and improves absorption of the other medicine at the same time.

Prevention and Botox
A preventive treatment is taken regularly — daily, or as a periodic injection — to reduce how often attacks come and how severe they are. It is not meant to stop an attack already underway. Options include medicines from several different families (beta blockers, some anti-seizure medicines, and some antidepressants at low doses — these last are prescribed for their effect on pain pathways, not because the headache is "psychological"), as well as the newer CGRP-targeted treatments.
Botox (botulinum toxin) is an approved preventive treatment specifically for chronic migraine. "Chronic" here has a precise definition: headache on 15 or more days a month, of which at least eight have migraine features, sustained over three months. It is given as a series of shallow injections into the muscles of the head and neck during a short session, repeated roughly every three months. Many patients respond from the very first session; in others the benefit accumulates gradually over successive sessions, which is why the effect is judged after two or three. This is a medical use of Botox, not a cosmetic one — the injection sites differ from cosmetic use — and it is not appropriate for episodic migraine.
Medication overuse: the hidden problem
Taking painkillers too often can turn episodic migraine into an almost daily headache, so it looks as though the condition has worsened when the medicine itself is the cause. The practical rule: if you regularly need a painkiller on more than ten days a month (or more than fifteen days for simple painkillers), tell your doctor. The answer is not a bigger dose but a plan to withdraw the painkiller gradually while starting a preventive treatment.
Practical steps that make a difference
- Keep your sleep times steady, including on weekends and holidays.
- Don't skip meals, and drink enough water through the day.
- Keep caffeine steady and limited rather than swinging between excess and abrupt withdrawal.
- Take regular, moderate physical activity; it has a proven effect on attack frequency.

- They interfere with your work, your studies or your daily life.
- Your usual painkillers no longer help, or you need them on many days of the month.

Signs that need emergency care now
- A sudden headache that reaches its peak within seconds (a "thunderclap" headache).
- Headache with fever and a stiff neck.

- Weakness or numbness down one side of the body, slurred speech, or confusion.
