متى يُطرح العلاج الوقائي؟
العلاج الوقائي دواء يُؤخذ بانتظام لتقليل عدد نوبات الصداع النصفي وشدّتها — لا لإيقاف نوبة قائمة. وهذه مهمة مختلفة تمامًا عن مسكّن النوبة، ولذلك يُؤخذ في أيام لا صداع فيها أيضًا.
ويُطرح عادةً في إحدى هذه الحالات:

ما الهدف منه؟
الهدف ليس اختفاء الصداع تمامًا، بل السيطرة عليه: تقليل عدد أيام الصداع في الشهر إلى النصف تقريبًا، وتخفيف شدّة النوبات، وتحسين استجابتها لعلاج النوبة الحادة، واستعادة أيامك.
ويحتاج أي علاج وقائي وقتًا كافيًا للحكم عليه: من ثمانية إلى اثني عشر أسبوعًا بجرعة مناسبة قبل أن نقول إنه لم ينفع. وإيقافه مبكرًا — بعد أسبوعين أو ثلاثة — من أشهر أسباب فشل العلاج، ومن أكثر ما يدفع المريض للاعتقاد أن «لا شيء ينفع معي».
خيارات العلاج الوقائي
الخيارات اليوم أوسع مما كانت قبل سنوات، وتشمل:
- أدوية يومية بالفم: من عائلات مختلفة — حاصرات بيتا، وبعض مضادات الصرع، وبعض أدوية الضغط، وبعض مضادات الاكتئاب بجرعات صغيرة. وهذه الأخيرة تُوصف لأثرها على مسار الألم، لا لأن الصداع «نفسي».

كيف تجري الجلسة؟

- أثر تجميلي ناتج عن حقن مقدمة الرأس، وهذا متوقع ومؤقت ويزول تدريجيًا.
- ونادرًا: ارتخاء مؤقت في الجفن أو الحاجب، يزول تلقائيًا خلال أسابيع.
أخبر طبيبك قبل الجلسة إذا
- كنتِ حاملًا أو مرضعة، أو تخطّطين للحمل.

When prevention is considered
A preventive treatment is taken regularly to reduce how often migraine attacks come and how severe they are — not to stop an attack already underway. That is a completely different job from an acute painkiller, which is why it is taken on headache-free days too.

Botox: who it is for
Botox is an approved preventive treatment specifically for chronic migraine. “Chronic” here is not a loose description but a precise definition:

- of which at least eight have migraine features,
- sustained over three consecutive months.
So it is not appropriate for episodic migraine (fewer than 15 days a month), and it is not used to treat an acute attack. It is usually considered after one or more other preventives have been tried, or where daily tablets cannot be used.

What a session involves

- Combine the medicine with steady sleep, meals and caffeine; the effect is cumulative.
